Called to ask if they had a form, they said yes. Went in the next day fir the form and the gentleman said yes and went straight down an aisle, grabbed the form and I purchased it. It was not until later that I gave the form to my boss that he said it was nowhere close to the form we needed... If the store easily replaces with the proper form I will add a star or 2 to my review....
